Is Wade on a basketball IQ decline?

Fast forward this year and I hate to say or even understand it but Wade is on the mental decline.

Has there ever been a player in history that forgot how to play basketball mentally?

correct me if i'm wrong but i'm pretty sure Wade has made this mistake before. Maybe heat fans can help me out but i think there might be one or two other times where he has missed the rim when intentionally missing a FT.
