In terms of effectiveness, which is what's most important, I'd list Shaq and Wilt above both. Those two basically required a double team in the low post if you didn't want them scoring on 70%+ efficiency. They didn't have the skill that those other two guys had, but they had more than enough size and strength to make up for it.

Came here to post this. Duncan has every shot and every move in the book. He can score effectively with either hand from both blocks. His footwork is impeccable.

A prime Wilt had entire seasons of near 40 ppg against Russell; entire seasons of 50 ppg against Bellamy; entire season of 40 ppg against Reed; entire season of near 30 ppg against Thurmond; an old Wilt had seasons of 24 ppg on .780 shooting against Lanier; and an old Wilt, in his last season h2h's against Kareem, outshot KAJ .737 to .450.
A prime Kareem had a season of 40 ppg against an old Wilt; 40 ppg against Cowens; an old Kareem had seasons of 30+ ppg on .630 shooting against Hakeem.
A prime Shaq had three straight Finals of 35 ppg on .600 shooting, and was dominating his peers for several years.
A prime McAdoo, in leagues with a near-prime Kareem, was outscoring everyone by solid margins, and outscoring KAJ in his h2h's. His post-seasons in his prime were well over 30 ppg, as well.
A prime Dantley had four seasons of 30+ ppg on .559 to .580 shooting, and his TS%'s were between .635 to .661.
Honorable mention:
McHale
Barkley
Moses (who just pounded his peers, including KAJ.)
Lanier
Gilmore
Hakeem
Duncan
